An incident report reveals the possible cause of a fire that killed a south strand man.

The fire started shortly after midnight Monday, at a home off Willbrook Road in Litchfield. 84-year-old Theodore Irwin died in the fire.

In a report from the Georgetown County Sheriff's Office, investigators say that his wife, Lois Irwin, says she decided to smoke a cigarette in bed, and that her oxygen tank caught fire, which caused the flames to spread to the rest of the house. It goes on to say that she repeatedly told doctors that it was her fault..

SLED is continuing to investigate the fire.
